What would make a lab report easier to discuss?

Hi, I am posting on behalf of AskDocDoc. I would like to hear from people working on patient information: what wording would you change on a lab report? I am interested in the explanation beside the numbers (for someone reading their own report), rather than a discussion of individual results.

Would you include a space for questions to bring to the next appointment? I would appreciate examples of wording you would use. Please leave out names and personal medical details.
